Detailed Description
If you completed your degree between 1993 and April 2004, it is not possible to replace your London Institute certificate. However, we are able to issue you with a ‘Pro-forma’ document that is printed on paper with the University crest and hologram and has the same information as provided by a certificate. (If you would prefer a letter confirming your award, please see ‘letters’ in e-store)
You are required to complete the attached form and take it to a legal representative to make a legal declaration that your certificate is lost, damaged, or destroyed. No requests for documentation will be actioned until this form is completed in full
Please see the URL below:
The cost for a pro-forma is £40 (including postage).
The delivery time is up to 15 working days from the receipt of the declaration form, payment and your details. Between June and July however, requests for proformas will take up to six weeks to process.
